Patients with cleft palates eventually require definitive fixed or removable dental prostheses after the maintenance of arch alignment and occlusal relationship during adolescence. This case report presents application of a resin composite veneered fixed partial denture utilized as a definitive prosthesis for a bilateral cleft palate patient after stable occlusion had been established orthodontically. The composite veneered long span fixed partial denture provides adequate aesthetics and function.  相似文献

The aim of this study was to examine the quality of written instructions and choice of impression trays and materials for removable partial dentures (RPDs) in the Kingdom of Bahrain. All six private dental laboratories in Bahrain were contacted and invited to participate in the study. Five laboratories participated, and submitted written instructions received by them for fabrication of both acrylic (A-RPDs) and cobalt-chromium (CC-RPDs) RPDs. These were examined for evidence of selected design variables. Types of impression trays and materials used were also recorded. One hundred and thirty-one written instructions were examined. Eleven percent (n = 14) were for CC-RPDs, 89% (n = 117) for A-RPDs. All treatments were provided on a private basis. Fifty-seven percent (n =1 8) of CC-RPD instructions requested the technician to design the prosthesis, 43% (n = 6) contained a diagram and 43% (n = 6) mentioned all design variables. Seventy-nine percent (n = 92) of A-RPDs requested the technician to design the denture, and only 1% (n = 1) mentioned all design variables. Alginate impression material was most commonly used for master impressions (83% of impressions (n = 109); 85% (n = 99) of A-RPDs, and 71% (n = 10) of CC-RPDs). Master casts were poured after a minimum of 24 h. Acrylic custom trays were used in 14% (n = 19) of cases (43% (n = 6) of CC-RPDs; 13% (n = 15) of A-RPDs). The quality of written instructions to dental laboratories for the fabrication of RPDs was found to be inadequate in Kingdom of Bahrain. There was widespread use of inappropriate impression trays and materials.  相似文献

In this prospective study, 120 teeth consisting of maxillary and mandibular canines and premolars were divided into three groups each containing 40 teeth. The teeth were assigned randomly in quadrants to three different periodontal treatment protocols. The first group was treated with professional prophylaxis only. The second group received additional deep scaling. With the third group, additional surgical periodontal flap surgery and scaling was performed. Both papillary bleeding index (PBI) and probing depth (PD) were evaluated before, during and after treatment. During the subsequent prosthetic treatment phase all teeth were then used as telescope abutments supporting a removable prosthesis. The documentation of the attachment level (AL) was then used as a clinical parameter. One year after the incorporation of telescopic removable partial dentures (RPDs), PD, PBI and AL were again evaluated. The resulting periodontal parameters were compared between the different groups using the general linear model (GLM) repeated measures and the Kruskal-Wallis test for non-parametric variables. Differences within the three treatment groups were determined using the t-test, e.g. the Wilcoxon test for dependent variables (P < 0.05). A significant decrease in inflammatory indices (PBI) was found for all types of periodontal treatment (P < 0.03 for all groups). Additionally, the reduction in PD was significant for all of the three groups (P < 0.001 for all groups). The greatest reduction in PD was observed in the group in which a surgical approach was used. Evaluation of the attachment level after the incorporation of the telescopic RPDs showed that tooth position did not influence the periodontal prognosis and that the use of telescopic RPDs exerted no ascertainable negative influence on the periodontium of the abutment teeth.  相似文献

Background: The replacement of a single tooth or several teeth by means of single‐implant restorations is an increasingly used method that needs long‐term validation. Purpose: The goal of this study was to evaluate the outcome of single‐implant restorations by means of fixed restorations and to define the prognosis through marginal bone level estimations. Materials and Methods: From November 1986 to June 1998, 270 Brånemark implants (215 in the upper jaw) were installed in 219 patients (106 males). Both anterior and posterior sites were involved. Of the 263 single restorations, 28 were placed in private dental offices. The patients were followed until June 1999. Results: Twelve implants failed before or at abutment connection or within 6 months afterward. Only four implants failed later. The cumulative success rates were 93% for the implants and 96.5% for the restorations over a period of 11 years. The marginal bone loss during the first 6 months after abutment connection reached 0.71 mm and then dropped to 0.036 mm annually over a period of 10 years. Conclusions: Single‐implant restorations (Brånemark System) are a reliable treatment with a good long‐term prognosis. Failures were concentrated during the healing period and early loading phase.  相似文献

目的:分析临床Co-Cr合金铸造可摘局部义齿(RPD)折断原因,以提高义齿质量。方法:对289件Co-Cr合金铸造RPD进行回顾总结,并对其发生铸造缺陷、折断进行统计分析。结果:本文289件铸件中,肉眼检查有铸造缺陷者93件,占31.3%,其余196件行X线探测检查有缺陷者36件,占18.6%。义齿各部位出现铸造缺陷机率:卡环小连接体处较高,下颌高于上颌。戴牙2年后折断32件,其中原铸造有缺陷者28件,占87.5%。结论:①铸造缺陷是造成铸造RPD折断的主要原因。②应用X线探测仪进行铸造义齿检测,是提高修复质量的有效方法。  相似文献

The original design of a semi-precision extra-coronal attachment for partial dentures is illustrated, and its fabrication in a cast non-precious bondable alloy is described. A step-by-step method is also described of bonding multiple attachments in parallel for the retention and support of removable partial dentures. Clinical cases are illustrated. The results of trials indicate that the method is clinically viable and, in selected cases, appears to offer distinct advantages over existing retention systems.  相似文献

目的:探讨夹板式可摘局部义齿在牙周病修复治疗中的疗效。方法:对67例患者应用铸造或弯制支架式可摘局部义齿,结合牙周病修复治疗的原则和方法,在修复缺失牙的同时固定松动牙。结果:经过修复后3年复查,有效率达92.5%。结论:夹板式可摘局部义齿是修复治疗牙周病较为理想的一种方法。  相似文献

目的:观察老年人三单位固定桥修复后的临床情况和对失败拆除原因的分析.方法:对近年来65岁以上老年人三单位固定桥修复进行5年至11年随诊观察,了解老年人固定义齿修复的特点,同时对该种固定义齿的失败拆除原因进行分析.结果:经过5年至11年临床观察,196件修复体修复后成功率为85.20%;失败拆除的原因主要是基牙牙周病,牙龈萎缩,食物嵌塞,修复体不密合,义齿脱落,义齿损坏,基牙继发性龋等.结论:结合老年人特点的短跨度固定义齿修复效果是肯定的,基牙牙周疾病是老年人固定桥失败拆除的主要原因.  相似文献

OBJECTIVES: This in vitro study was aimed to compare the fracture resistance of directly fabricated inlay-retained fiber-reinforced composite (FRC) fixed partial dentures (FPDs) with four types of framework designs. METHODS: Forty-eight directly fabricated inlay retained FPDs were made of FRC and particulate resin composite (everStick/Tetric flow and Ceram). Extracted human mandibular first premolars and first molars were as abutments. The following framework designs were tested: in the Group A (control group), the framework was made of two prepregs of unidirectional glass FRC; the Group B, two prepregs in pontic portion were covered with one layer of multidirectional fiber veil FRC; the Group C, the FRC prepregs were covered in pontic portion with four short unidirectional FRC pieces along the main prepregs; in Group D, one short unidirectional FRC prepregs were placed on the main prepregs in 90 degrees angle to the main framework. After thermal cycling, FPDs of each group (n=12) were randomly divided into two subgroups (n=6). Fracture test was performed at the universal testing machine (1mm/min) where FPDs were loaded from the occlusal direction to the occlusal fossa or to the buccal cusp. Failure patterns were observed with stereomicroscope. Median and 25%/75% percentile values were calculated and nonparametric analysis was performed. RESULTS: Compared with three other framework designs, the FPDs in Group D showed the highest resistance when loading to the occlusal fossa, with maximum load of 2,353.8N (25%/75%: 2,155.5/2,500.0) (p=0.000, 0.000, and 0.005 for compared with Group A, B, and C). The same group showed also higher resistance when loaded to the buccal cusp (1,416.3N (1,409.2/1,480.8)) if compared to the FPDs of the Group A and Group C (p=0.044, 0.010). In general the FPDs showed higher resistant to loading at the occlusal fossa (p<0.05). CONCLUSIONS: This in vitro study showed that inlay-retained FRC FPD constructed with direct technique provided high fracture resistance. The framework design that provided support for the veneering composite of the pontic contributed to the highest load-bearing capacity even when loaded to the buccal cusp.  相似文献
